ASSUMPTION OF RISK: I, the undersigned, acknowledge that participation in basketball training, camps, leagues, open gym sessions, and related activities ("Activities") at FCP Sports involves inherent risks of physical injury. These risks include, but are not limited to, sprains, fractures, concussions, ligament tears, muscle strains, heat-related illness, and in rare cases, catastrophic injury or death. I voluntarily assume all such risks.
RELEASE OF LIABILITY: In consideration of being permitted to take part in Activities offered by FCP Sports, its owners, officers, coaches, employees, volunteers, and agents (collectively "Released Parties"), I hereby release, waive, discharge, and agree not to sue the Released Parties from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in Activities, whether caused by negligence of the Released Parties or otherwise.
MEDICAL AUTHORIZATION: In the event of an emergency, I authorize the Released Parties to secure medical treatment, including transportation to a medical facility. I understand that I am financially responsible for any medical expenses incurred.
MEDIA RELEASE: I grant FCP Sports permission to use photographs, video recordings, and likenesses taken during Activities for promotional, educational, and marketing purposes without compensation.
RULES & CONDUCT: I agree to abide by all rules, regulations, and instructions of FCP Sports coaching staff. I understand that FCP Sports reserves the right to dismiss any participant whose conduct is deemed unsafe or disruptive, without refund.
GOVERNING LAW: This waiver shall be governed by the laws of the State of Florida. Any dispute arising under this agreement shall be resolved in the courts of Okaloosa County, Florida.
SEVERABILITY: If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.
